Evolve Med Inc, is a non-profit organization and is committed to promote health and well-being to the local population targeting children, elderly and low-income families; promote and encourage cooperation and collaboration with other organizations within the community who seek the same or similar goals. Our focus is on preventive care and health education to promote healthier lifestyles in the communities we serve.
Our services are available to families, businesses and individuals regardless of age, race, sex, sexual orientation, national origin, or their ability to pay. Our medical staff consists of highly-trained board-certified nurse practitioners, LPNs, Medical Assistants, and other support staff.
